Dental Plan Information – If you enrolled in The Standard Dental plan, you should have received via (US Mail) two ID cards with The Standard logo – one for your Dental Coverage, and one for your embedded EyeCare coverage. This Eyecare coverage ID card is a reminder of the $150 vision benefit that is provided with your Dental plan. To use this added vision benefit, you will need to submit a claim reimbursement form with The Standard. Please keep in mind, if this benefit is used, the $150 will be deducted from your Dental plan’s annual calendar year maximum.

Below are generic temporary ID cards that can be used if you have not received the mailings from The Standard or had a chance to register above.

Vision Plan Information – If you enrolled in the stand-alone Vision plan, your Vision ID card will have both The Standard and the VSP logos on it. You will use this card when you want to access your full vision benefit plan. This generic temporary card can be used at the time of service if you have not received the mailings from The Standard or had a chance to register above.

NOTE: If you enrolled in both the Dental and Vision plans through The Standard, you should have received a total of 3 ID cards.

Health Spending Accounts (HSA)

HEALTH SAVINGS ACCOUNT (HSA) –  Group Dynamics, Inc.
An HSA or Health Savings Account is an account that an employee can set up to cover qualified health care expenses. Unlike the FSA (Flexible Spending Account), this is a personal account and can be used after the employee leaves the College. This account must be paired with a Qualified High Deductible Health Plan (QHDHP).  Employees can contribute to this account on a pre-tax basis. The IRS sets annual maximums for contributions to these accounts (www.irs.gov).

  • Setting up your HSA account with Group Dynamic (GDI):
    • Following election of an HSA health plan in PlanSource, you will need to create an HSA account with GDI. Please go to https://gdiconsumer.lh1ondemand.com/Login.aspx?ReturnUrl=%2f and follow the instructions to create your account.
    • Although the process to open your account is easy by completing each screen, detailed instructions are available in this QuickStart Guide from GDI.
