Once you have obtained a preceptor for your clinical/practicum experience, you must submit your information to the Clinical Placement Team through the online submission form.

CHECKLIST FOR GATHERING PRECEPTOR AND FACILITY INFO:

The link below provides you with a check-list of the information you need to enter into the online submission form (also available through a link on the form). The checklist may be helpful in assisting you in collecting the information prior to entering this into the online form.

Please remember that all preceptor forms and resumes must be submitted by the SUBMISSION DEADLINE to be processed. Any paperwork received after this deadline will be processed for the following term. All CastleBranch requirements and Affiliation Agreements with the sites need to be completed/approved by the COMPLETION DEADLINE.

Deadlines for Submitting Preceptor and Site Info for Approval and Clearance to Register for Clinical/Practicum Course

STEP 1:

Submit your preceptor application by the deadline below for your desired term using the online form.  IMPORTANT:  Please make sure you have provided accurate information for the preceptor AND for a contact person within the healthcare organization who coordinates placements/affiliation agreements. Inaccurate information can delay the review and processing for your placement.

STEP 2:

Ensure that all requirements are completed including CastleBranch (immunizations, drug test, background check, and other required documents) and any additional requirements your clinical site may require.

*Please note that if you are unable to submit your preceptor/site information by the deadline, we will try to accommodate later submissions as time allows, but due to review and processing timeframes we cannot guarantee that late submissions will be finalized in time for your desired term. Please contact Nursing Operations Administrator at cmoor@sjcme.edu  if you need an extension.

PLEASE NOTE: Affiliation Agreements can take anywhere from 1 week to several months to finalize and we cannot allow students to participate at any clinical site unless there is a current Agreement in place. Although all submissions meeting the deadline will be processed, we are not able to guarantee that the Agreement will be finalized by the COMPLETION DEADLINE so we encourage students to submit their preceptor/site information well in advance of the SUBMISSION DEADLINE.

You will be notified of clearance for your clinical/practicum within 7 days of the COMPLETION DEADLINE.

Health and Background Requirements

All nursing students are required to purchase CastleBranch in order to track immunizations and other documents required to be in compliance with SJC and clinical site policies. Your CastleBranch package includes:  Immunization/Document tracker, drug test, background check and your SJC Nursing Badge.

Maine Clinical Site Resources

AR Gould Hospital:
Advanced Practice students interested in completing a clinical rotation at AR Gould Hospital will need to complete an application and review by the Medical Executive Leadership Team. Please advise your students to contact our Clinical Education Department to initiate this process.

Students are not permitted to independently seek out a specific provider and they must adhere to the process to assure that all regulatory requirements are met. Failure to follow the above process may forfeit the opportunity to complete a clinical rotation at AR Gould Hospital.
