Student insurance waiver notice instructions

1. Log in at My Web Services
2. Username/Password is same as your email. Contact IT (207-893-7851) for assistance.
3. Click on My Info
4. Click on Web Services
5. Click on Health Insurance Acceptance/Waiver
6. Follow the instructions to complete the waiver process. It is mandatory that a copy of the front and back of your insurance card be submitted to Health and Wellness Center. Submit card to this encrypted google form.
